I, the undersigned, understand and agree to the following terms and conditions related to the enrollment and participation of my pet(s) in Dog Star:
1. Assumption of Risks:
I understand that dog daycare, boarding, walking, and in-home pet care involve inherent risks, including but not limited to injuries from rough play, bites, or other dog-related incidents.
2. Health and Vaccinations:
I certify that my pet(s) is in good health, up-to-date on vaccinations, and free from contagious diseases. I will promptly inform Dog Star of any changes in my dog's health status, including but limited to exposure to Kennel Cough, Giardia, or Canine Influenza.
3. Behavior and Training:
I am responsible for informing Dog Star of any behavioral issues or sensitivities my dog may have. I will promptly notify the facility of any changes in my dog's behavior or training status.
4. Emergency Veterinary Care:
In the event of illness or injury, I authorize Dog Star to seek veterinary care for my dog. I understand that I am responsible for any associated veterinary expenses for my pet or any my pet may have harmed; I further understand that Dog Star will try to use my veterinarian but may use a different veterinarian at their discretion.
5. Release of Liability:
I hereby release and discharge Dog Star, its owners, employees, and agents from any claims, liabilities, costs, expenses, or judgments arising from any incident or injury involving my dog while in the facility's care.
6. Indemnification:
I agree to indemnify and hold harmless Dog Star, its owners, employees, and agents from any claims or actions by third parties arising out of my dog's behavior or actions.
7. Photography and Media:
I grant permission for Dog Star to use photographs or videos of my dog for promotional or educational purposes.
8. Cancellations and Fees:
I understand the facility's cancellation policies and fees, as outlined in the facility's terms and conditions.
I have read and understand the terms of this Dog Daycare and Boarding Liability Waiver.
